Report a Personal Data Breach

หากท่านพบหรือสงสัยว่ามีข้อมูลส่วนบุคคลรั่วไหล ถูกเข้าถึงโดยมิชอบ ถูกแก้ไข สูญหาย หรือถูกนำไปใช้โดยไม่ได้รับความยินยอม โปรดแจ้งผ่านแบบฟอร์มนี้
If you find or suspect that personal data has been leaked, accessed without authorization, altered, lost, or misused, please report it using this form.

ขั้นตอนหลังจากท่านแจ้งเหตุ / What happens after you report

  1. เจ้าหน้าที่ผู้ประสานงานของวิทยาลัยนานาชาตินวัตกรรมดิจิทัล (ICDI) จะตรวจสอบข้อเท็จจริงและประเมินความเสี่ยงของเหตุการณ์
    The ICDI coordinator will verify the facts and assess the risk of the incident.
  2. หากประเมินแล้วพบว่ามีความเสี่ยง วิทยาลัยจะส่งเรื่องต่อไปยัง ศูนย์ส่งเสริมการคุ้มครองข้อมูลส่วนบุคคลและการธรรมาภิบาลข้อมูล มหาวิทยาลัยเชียงใหม่ (PDPG CMU) เพื่อพิจารณาดำเนินการตามกฎหมาย รวมถึงการแจ้งสำนักงานคณะกรรมการคุ้มครองข้อมูลส่วนบุคคล (สคส.) ภายใน 72 ชั่วโมงนับแต่ทราบเหตุ
    If a risk is identified, the case will be escalated to the CMU Personal Data Protection and Data Governance Center (PDPG CMU), which will take further legal steps, including notifying the PDPC within 72 hours of becoming aware of the breach.
  3. กรณีมีความเสี่ยงสูง เจ้าของข้อมูลที่ได้รับผลกระทบจะได้รับแจ้งพร้อมแนวทางเยียวยาโดยไม่ชักช้า
    In high-risk cases, affected data subjects will be notified without delay, together with remedial measures.
